This working paper presents the LumenFold Protocol (LFP-0), a minimal semantic accountability layer designed for high-risk AI systems such as autonomous driving (FSD) and large tool-using models (e.g., Grok-like stacks). Instead of modifying model weights, LFP-0 operates at the decision boundary and adds a thin semantic audit layer: a six-field JSON header plus a single ambiguity score H for each human–AI decision event. The protocol makes it possible to later reconstruct who decided what, under which level of ambiguity, based on which evidence, what action was actually committed, and how the decision can be challenged. We describe the motivation from real-world failure modes, the formal structure of the 6+H fields, the notion of “contested folds” for borderline decisions (H in a mid-range band), and structural simulations showing that the logging overhead remains under approximately 3–4% with no serious tail-latency spikes. We also discuss how LFP-0 can be integrated as a Layer 0 “fuse” beneath higher-level alignment methods, and how it relates to emerging practices in AI governance, accident investigation, and scalable oversight. This record defines LumenFold Protocol (LFP-0) as a minimal, implementable logging and governance standard for semantic accountability in high-risk human–AI decision systems.
