Augmented Reality (AR)-assisted healthcare exercise systems offer innovative solutions to enhance healthcare outcomes. However, the reciprocal influence between the design features of these systems and patients’ continuous intentions has received limited attention. Based on the Technology Acceptance Model, this study develops a framework to investigate the relationship between three design attributes (interactivity, authenticity, vividness) on continuous intention, considering utilitarian and hedonic factors as mechanisms and health consciousness as a moderator. A questionnaire was administered to 295 inpatients at a large academic medical center in China. The analysis followed a dual-stage approach: (1) Partial Least Squares Structural Equation Modeling (PLS-SEM) elucidates perceived interactivity directly affects perceived ease-of-use and playfulness, perceived authenticity directly affects usefulness, and perceived vividness enhances playfulness. (2) Fuzzy-set Qualitative Comparative Analysis (fsQCA) reveals that while perceived authenticity is not a necessary condition for high continuous intentions, it becomes sufficient when combined with other variables. These findings suggest that healthcare providers and system developers should prioritize optimizing patient interaction and visual engagement to create effective experiences.
