Deep Diving into Blockchain Frameworks and their Issues

M Sowmya, Rajesh Kumar Kaushal, Shanthi Makka · 2025

The healthcare industry within modern digitalized systems deals with four main problems related to privacy standards alongside integration flexibility and lightweight authentication systems for Digital Health Record (DHR) management. Blockchain technology serves as a promising solution because of its security features along with features of patient-centeredness and decentralized architecture. The paper investigates Hyperledger Fabric as a permissioned DLT which allows controlled health data access through its low-energy PBFT algorithm implementation. Its design base enables connection to fog computing systems and Web 3.0 applications as well as Internet of Things devices without restriction. The deployment of this system faces several practical application problems alongside challenges regarding addressable complexity and extreme processing demands. Various potential solutions for DLT implementation include side chaining protocols in addition to sharding solutions and complex real-world platform integration projects. The domain of healthcare features four distinct upcoming advancements represented by Multimedia healthcare together with smart hospitals and blockchain-based telemedicine and intelligent data management systems. The authors demonstrate how Hyperledger Fabric could lead to better patient results through a healthcare structure that provides enhanced security and interoperability and effectiveness. The article studies Permissioned DLT Hyperledger Fabric which enables controlled healthcare data access utilizing a PBFT algorithm that conserves energy.
