The transparency, decentralization, and immutability of blockchain technology offer a revolutionary and secure force in health care. Blockchain, as a technology, ensures integrity by distributing information across all nodes in the network, confidentiality of sensitive medical data by decentralization that promotes trust via access control systems, and secure exchange of medical data. It facilitates transparent data management and improves the quality of care during diagnostic and treatment procedures. Additionally, blockchain&s;s traceability feature provides immutability, allowing any illegal modification of medical information to be identified and therefore maintaining data integrity. In this chapter, we present Perforated Blockchain, a smart health-care architecture that combines blockchain technology with the concepts of trigger as a service and perforated cards to build a block. Our contribution is divided into two phases: pre-processing as well as processing and the follow-up phase, which make use of triggering and the supervised classification approaches. During the preparation step, patients register and the blockchain is set up, similar to preparing a stack of perforated cards. In the treatment and follow-up phase, patient-reported symptoms are used to select the most appropriate treatments depending on their condition. Following therapy, doctors confirm the patient&s;s recovery status by updating the information, much like perforating cards in a stack. The idea presented in our contribution in this chapter combines blockchain&s;s security and immutability with the notion of perforated cards to provide a solid solution for efficient health-care administration. This architecture solves critical difficulties in health care by combining the capabilities of blockchain technology with trigger-based classification, guaranteeing data integrity and confidentiality for better patient outcomes.
