larger enclosed loop area.Notomi et al. 3 speculated that the area enclosed by the loops may indicate potential energy stored during systole, similar to a wound-up spring. This potential energy is converted into kinetic energy during early diastole, similar to the release of the spring, resulting in higher untwist rates. This early diastolic phenomenon in children may spare the left atrium from significant rise in pressure and may explain why children with HCM experience less exercise-related symptoms compared to adults. Our study was conducted in a single center, serving as a pilot study.A larger study on LV rotational mechanics with exercise capacity is needed in children. With increasing application of automation in medicine, the ''physiologic'' indices proposed by us may serve as ''clinical'' indices, if incorporated into future versions of ultrasound machines, and may gain acceptance in busy clinical settings.
