In recent times, the Internet of Things (IoT) has emerged as a significant factor in various sectors, encompassing industries, healthcare, urban development, residential environments, autonomous vehicles, agriculture, and energy distribution, among others. The smart healthcare system (SHS), an IoT-based healthcare system, is experiencing a surge in popularity. This can be attributed to various factors such as the scarcity of medical resources, the growing population of elderly patients with chronic illnesses who require remote monitoring, and the escalating expenses associated with healthcare. The Internet of Medical Things (IoMT) represents the latest phase of the IoT and has garnered significant attention from the scientific community for its extensive application in smart healthcare systems. The integration of intelligent wearables and sensors within a smart healthcare system has facilitated the flow of data, leading to the introduction of advanced real-time patient monitoring and an improvement in the overall quality of medical care. Nevertheless, despite the extensive participation of SHS in the provision of comprehensive medical services, it is important to acknowledge that these services are not devoid of risks and vulnerabilities, particularly in relation to matters of security and privacy. Smart healthcare systems utilise wireless networks and information technology to facilitate the interchange and analysis of patient data. If the security and control measures of the smart healthcare system are insufficient, it becomes vulnerable to compromise by attackers. This vulnerability presents an opportunity for attackers to inflict harm upon patients, potentially leading to fatal consequences, all while remaining undetected. The intrinsic attributes of intelligent healthcare systems, such as their capacity for expansion, intricate nature, and diverse range of devices, pose significant challenges in promptly identifying and safeguarding against such cyber threats. This chapter endeavours to offer a methodical and all-encompassing examination of the security and privacy concerns linked to Smart Healthcare Systems (SHS), as well as the security solutions put forth by the research community to safeguard SHS. Ultimately, the chapter culminates by presenting many prospective avenues for future research within the realm of safeguarding Internet of Medical Things–based intelligent healthcare systems.
