When a scratch org is created, a user called User User is automatically added that has the System Administrator profile assigned to it. If your application or customization is only used by system administrators, you can perform testing with this user. Having said that, you will normally want to perform testing with different end users in mind, such as service agents and account executives; therefore, it makes much more sense to test with a user that has the same permissions as the end users in your production org.
