Understanding Why Products Fail

Products can be tested or operated to failure. When a product has failed, the failure mechanism must be learned to determine the root cause of the failure. The design of the product or the process must be updated to remove the failure possibility from happening. Nothing has been more important than the quality of the components that go into a product. Much work has been done over the past few decades to improve the quality and reliability of components. There has been a paradigm shift where software quality escapes often significantly outnumbers the number hardware reliability issues. There are several software process improvement models an organization can select from to improve software quality. The models help an organization evaluate their current software quality process, infrastructure, and capabilities and they also provide a framework on how to make incremental improvements. The most common software quality improvement models are the Capacity Maturity Model and ISO 9001: 2015.
