In 2011, Project Management (PM) Solutions conducted a survey to identify the top causes of information technology project failure. e report, called, “Strategies for Project Recovery,” covers 163 companies of dierent sizes, which manage on average $200 million in projects each year. e study identies the ve top causes of troubled projects (PM Solutions 2011): 1. Requirements. Unclear, lack of agreement, lack of priority, contradictory, ambiguous, imprecise 2. Resources. Lack of resources, resource conicts, turnover of key resources, poor planning 3. Schedules. Too tight, unrealistic, overly optimistic 4. Planning. Based on insucient data, missing items, insucient details, poor estimates 5. Risks. Unidentied or assumed, not managed Tables 4.1-4.5 break down each of these failure points and show their alignment with the guiding principles of a consultative approach, which should be used to mitigate these risks or would have avoided these failures on programs altogether.
