Just about everyone is accustomed to hearing about attacks on systems; often these are the result of attackers exploiting known weaknesses, missing patches and poorly implemented security policies. Sometimes they are carried out by disgruntled employees, sometimes an employee will, in total innocence, take some action that could lead to systems being compromised.
