Research on the Design of Community Health Management Service for the Aged under the Background of Medical- nursing Combined Service in Big Data Era

In view of the severe situation in the age of aging in China, the pressure of family and society for the aged continues to expand, and there are home nursing service, community nursing service and other pension service modes, but these pension models are not perfect enough to meet the needs of the elderly. Through the analysis of the present situation of pension mode in China, the demand of community pension model of medical support combination is analyzed, and the problems of existing in the current community pension service system have been combined, and a new mode of medical care combined with community care service is proposed, the community health management service system for the elderly under the mode of medical nursing combined service has been constructed in line with China's national conditions, it’s to meet the future development needs of the elderly. That, under the background of healthy China, a community elderly care service platform combining medical care with nursing care has been established, and a sound, diversified, intelligent and humanized community elderly care health management service has been provided.
