Reducing a disease to one or a few mutated genes and adjusting the medicine to the requirements of a specific patient carrying these mutations means basically an individualizing approach. A medieval pharmacist preparing a specific medicine for a patient took part in personalized therapy. Optimized target selectivity will be the strategic research goal of a novel cell function-based combination therapy, which is a more straight forward approach in comparison with the attempt to design multitarget activity into a single active pharmaceutical ingredients (API). Optimizing of pharmacokinetics of established APIs to reach the desired target organ or tissue is a further field for a variety of smart complementary approaches for innovation in drug research. Biomedical research breaks down diseases from symptoms and established diagnosis to the cellular level. The more detailed knowledge on diseases together with the increase in available patient data allows novel gene- and biomarker-based optimization of patient recruitment for clinical studies.
