Insider attacks: human-factors attacks and mitigation

For many organisations, the human element is generally the weakest link and often the reason why many security incidents occur. As an organisation grows, the chance of a human-caused security incident increases exponentially. Even given the most security-oriented individual, it only takes one slip-up, a minor mistake, or someone having a bad day to create a security incident.
