INFLUENCE OF THE TERMS OF ATOPIC DERMATITIS ONSET ON CORTISOL SECRETION

Objective – to compare peculiarities of basal cortisol secretion in adult AD patients depending on the disease onset term and total IgE secretion level.Material and methods. 63 adult patients with AD have been examined and divided into 4 groups depending on the disease onset term and total IgE level. AD severity and blood cortisol levels were determined at hospital admission, discharge and following one month of observation in the outpatient setting. The obtained data were compared with those from healthy persons and between the groups in dynamics.Results. Decrease in the morning cortisol secretion at high relapse of AD was determined to be more natural to AD patients with the disease start in early childhood. Dynamics of cortisol secretion level under the treatment influence was significantly high in patients with high levels of total IgE. Relationship between morning cortisol level, and AD severity, clinical variant, as well as age or sex was absent.Conclusions. Signs of hypothalamus-pituitary-adrenal axis depression are more pronounced during exacerbation in AD patients with the disease onset in early childhood than in patients with the late AD onset. Standard inpatient treatment is accompanied with the rise of cortisol level in IgE-dependent AD patients. The results obtained are necessary to select AD treatment strategy.
