Analysis of First Symptoms and Emergency Diagnosis of Aortic Dissection

Objective To understand the emergency symptoms of aortic dissection,in order to reduce misdiagnosis.Methods We retrospectively analyzed the emergency symptoms,auxiliary examination results,and clinical diagnosis of 21 patients treated between January 2006 and July 2012.Reasons for misdiagnosis were summarized.Results Emergency symptoms included intense chest pain and/or abdominal pain in 15 patients(71.4%),chest distress and dyspnea in four patients(19%),chest pain with hemiplegia in one patient(4.8%),and painless syncope in one patient(4.8%).Fifteen patients took ECG examination,and the results were abnormal in 10(66.7%);seven in 17 patients who took chest X-ray examination were abnormal(41.2%);false cavities in aorta(100.0%) were found in all nine patients who took enhanced spiral CT examination;ten patients took UCG,and false cavities were found in eight(80.0%);fifteen patients took creatine kinase MB,Myoglobin,and Troponin I examination,and four were abnormal(26.7%);twelve patients took the measurement of D-Dimer,and three showed abnormalities(25.0%).Fifteen cases were diagnosed as aortic dissection(71.4%),and others were misdiagnosed.Conclusion The clinical manifestations of aortic dissection are complicated,and enhancing vigilance and proper auxiliary examinations are helpful for early diagnosis.
