This paper is to discuss the research problems of multimedia database（MMDB）based on hierarchy-relational（H-R） model．First, the new idea of hierarchy and segment dealing with multimedia data is expounded，so that a H-R model of hierarchy storage structure and relational data manipulation is founded．Then，the typical instance implement using with refined B+ tree i.e. RFN-B+ tree is given．The presented design idea is of practicing value after the software exploitation testing．
