The major existing methods and their shortcomings about human errors in civil aviation industry were summarized in order to analyze human errors of pilots systematically.A new human error model for pilots was built from the micro-cognitive processes and macro-HFACS framework based on pilot-information-processing and accident-chain principle. The kernal modules of this model, such as scenario analysis, error patterns and classification, cognitive mechanism, error recovery, error causation, feedback mechanism and protection mechanism, were introduced. The key factors of the cognitive mechanism generated by pilot errors were investigated. The steps and thoughts for structured error analysis were put forward. The model is to provide a new method for the analysis of cognitive mechanism, classification and causation of pilot errors, however, its farther practical application needs to be validated by flight accident analysis.
