Purpose: To investigate the diagnostic potential and accuracy of an automated breast volume scanner (SomoV™) in comparison with handheld ultrasound (HHUS) and mammography referring histological confirmation as standard.. Patients and methods: Subgroup analysis of 203 patients – out of 1430– with 248 breast lesions. All 203 patients had mammograms, HHUS and an automated three dimensional US breast volume scan. Each patient underwent an US or mammography guided core needle biopsy for histological confirmation. The data were evaluated according to the breast imagining reporting and data system (BIRADS) classification. The physician investigating the SomoV™ images was blinded to all patient variables and to all other diagnostic results. Results: 152 lesions (61,3%) were confirmed as malign and 96 lesions (38,7%) as benign. The sensitivity of mammography with 86,8% was greater than of both HHUS (81,6%) and SomoV™ (53,3%). The positive predictive value of defining a suspicious lesion and biopsy recommendation was for HHUS (69,7%) and for mammography (69,1%) lower than for SomoV™ (88,5%). There was no significant difference between the diagnostic accuracy of each method. Conclusion: An automated breast volume scanner is a promising diagnostic tool in addition to HHUS and /or mammography 3D ultrasound - SomoV™ - automated breast volume scanner - mammography - manual US
