Considering the importance of the patient's medical information for the caregivers to ensure that patients receive the appropriate treatment in light of his medical history, sharing distributed medical information among caregivers is essential. On the other hand, these caregivers suffer from communication gaps and the heterogeneity of their medical records. As a solution, some past works have proposed standardization of medical records. Others proposed techniques to achieve partial automation of interoperability based on medical record schema or semantics. This paper proposes MREx, an ontology mapper in Medical informatics, which enables caregivers, with electronic medical records, to share real-time comprehensive medical information on demand, in time, and at the point of care. MREx uses both schema-level and instance-level matching. It functions with no data centralization, in a complete distributed heterogeneous environment, and without having to replace existing EMRs. The system shall allow caregivers to serve inter-institutional patients, provides staff with real-time information to make critical decisions and serve researches to collect data among all caregivers.
