Pelger-Huët Anomaly and Megaloblastic Anemia

A patient with folate-defirient megaloblastic anemia of pregnancy and coincident Pelger-Huët anomaly of leukocytes is presented. Prior to oral folate treatment, neutrophilic nuclei with three, four, and five segments were seen in the peripheral smear. After treatment, only the characteristic bilobed and “indented rod” nuclei were seen. A family study verified the presence of the Pelger-Huët anomaly. It is confirmed that Pelger-Huët subjects react in the usual way to the megaloblastic process by “hypersegmenting” neutrophils.
