Exploring the role of diagnosis in software failure

Among engineering systems, software systems are unique in thier tendancy toward repetitive failure, and the situation is worsening as systems become larger and more tightly coupled. To stem the tide, the author advocates a humbler attitude toward software failure and improved diagnostics.
