Principles of target-oriented construction of medical questionnaires

Medical history questionnaires, which may include the use of computers for acquisition, presentation, and evaluation of the data, support a restricted spectrum of the functions of the anamnestic process. Some relevant approaches are reviewed. A broad variety of alternative technical solutions have been tried and in particular the simple techniques are used on a broad scale. The acceptability of these devices by patients and physicians and the usefulness of the procedure in the process of medical care have been assessed for relatively few of these. The available evidence shows that patients are able and willing to use the devices, but physicians tend to be reserved as to their value.The data on medical usefulness help to explain this discrepancy: the questionnaires tend to pick up too much and too unspecific data. The results of related experiments are summarized.A modification of the strategy of questionnaire construction is suggested to reduce these shortcomings. Instead of starting from a list of possible symptoms one might ask for, it is suggested to start from a specification of the conclusions one wants to arrive at. This process is called ‘target-oriented construction’ since it sets a goal. The method is in accordance with basic principles for the design of information systems.Les questionnaires automatisés permettent l'acquisition, la présentation et la validation des informations concernant les antecedents et l'histoire de la maladie, mais sont assez contraignants pour l'anamnèse. Plusieurs approches intéressantes sont passées en revue. Différentes solutions ont été essayées. Ce sont les techniques les plus simples qui sont les plus répandues. L'acceptation de ces moyens par les malades et les medecins et leur utilité n'ont été réellement évaluées que clans un petit nombie de cas. Les patients acceptent volontiers de se prêter à ce genre d'interrogatoire, mais les médecins restent assez réservés à son égard. Cette divergence est due à ce que les données recueillies sont très nombreuses et peu pertinentes. Pour remédier à cette insuffisance, l'autcur propose de modifier la stratégic de construction des questionnaires. Au lieu de partir d'une liste de symptômes, on pourrait se centrer sur les spécifications des conclusions auxquelles on veut aboutir. Ce procédé est appelé construction orientée vers une cible. La méthode a été testee. Ses résultats montrent qu'elle apporte une amélioration par rapport aux précédentes techniques d'interrogatoire.
